This comprehensive assessment provides in-depth information about your child’s level of independence and support needs. This assessment is recognised by the NDIS to support funding requests (ie. request to become an NDIS participant, request for increased funding, request for assistive technology, request for support worker hours etc.).

Age: 0-16 years

Assessment package involves:

• Online questionnaire for caregivers (to be completed before assessment day)

• 1x 2-hour appointment OR 2x 1-hour appointments

• Report (takes up to 8 hours)

• Discussion of results via phone call (optional)
